Who this notice covers
Atticus Health, Inc. ("Atticus", "we", "us") furnishes care-management and care-coordination services directly to the individuals enrolled in our programs. For that reason, Atticus is a HIPAA covered health care provider, and this is our Notice of Privacy Practices under the HIPAA Privacy Rule (45 CFR § 164.520).
This notice applies to the medical information we create or receive about you in connection with care management: your records, medications, lab results, hospital and visit events, assessments, and the care activity we manage on your behalf.
How we may use and disclose your health information
For treatment
Our purpose is treatment. We coordinate your care directly, working alongside the clinicians and providers who treat you, and share information with your care team so they can act on what we find. When we request records from health information networks on your behalf, we request them for this treatment purpose.
Payment and health care operations
As HIPAA requires every covered provider to disclose: we may also use a limited amount of health information to bill for the care-management services we provide directly to you, and for our own internal quality and safety review. We do not request or use information from health information networks for payment or health-care-operations purposes on behalf of any other organization.
With your family circle
We share health information with the people you invite into your family circle, only to the extent you choose to share with each of them. You control this in the app and can change it at any time.
Other permitted or required disclosures
We may use or disclose your information without your written authorization when the law requires it — for example, for public health activities, health oversight, judicial or administrative proceedings, law enforcement purposes, to avert a serious threat to health or safety, or as otherwise required by law. We disclose only what the law requires or permits.
Uses that require your authorization
Any use or disclosure not described above requires your written authorization. If you give authorization, you may revoke it in writing at any time, except to the extent we have already relied on it.
We never sell your information, and we never use or disclose it for marketing or advertising.
